The above forms are included in our New Patient Registration Pack that can be or collected from reception or downloaded from the links below.


As part of the registration process we will require you to show two forms of identification from you. One of these must be photographic. These can include a British Citizen Passport (or document issued and endorsed by the Home Office confirming right of residence in UK), driving licence (both parts), Home Office Application Registration Card, birth/marriage certificate, utility bill with current address on it, and bank statement with current address on it.


When registering a new baby, please bring the registration form you received when registering the birth.


All our patients are registered with the Biggar Medical Practice and not with an individual doctor.


If your details change at anytime you should inform us by completing the change of details form below and email it to the practice.


If you are residing in the practice area for less than three months and need to see a doctor you will need to register as a Temporary Resident.  You can download the form by following the link below.


To book appointments and order a repeat prescription online you will need to complete a registration for online services form which can be downloaded from the link below.  However, please note we cannot complete your registration for online services until we have received confirmation of your registration and your NHS Scotland Number (CHI Number).  This takes up to 5 days from submitting your registration forms.
